Palmetto Region Officiating Concern Remittal

This is for the purpose to remit any concerns or complaints toward a officiating team and/or person.  These are taken seriously and should be valid remittals.  Only the head coach of a team or Club director may remit an entry. 

It is to be used for when you have poor officiating and/or scorekeeping.

  Name and date of the tournament

  Your Name

Your Club and Team Name

  Your position with the team/club ( coach, Asst coach, director)

 
Club and Team Name of who was officiating and complaint being filed against ( needs to be correct- look on your tournament format, also provide Team  11 digit code)

  Name of person who concern is being remitted

  This person's duty on the officiating team? ( R1, R2, Line, scorebook, flipper, other)

Briefly describe in as much detail as possible the complaint- supply exact details- exactly what was done or said- by whom. 

Provide any direct witnesses to the incident that can support the information. Name, phone # and Email address.
