NEW Junior Players ONLY

Step By Step Instructions

(Not a member last season)

 

1.  PRINT, READ AND FOLLOW ALL DIRECTIONS.

2.  If you were a member of a region other than the Palmetto Region last season please contact your NEW Region registrar to transfer your membership PRIOR to going online to register. Send your name, date of birth and address to:  pvaheather@aol.com

3.  Go to the web address below or press on the link:

4.  Membership Information page will come up. Fill out all fields on this page, fields marked with an * are required fields. Hit the “Continue” button when completed.

5.  Membership Option page will come up. Choose the club you plan to tryout for and then hit the “Tab” key.   MAKE SURE IF YOU ARE TRYING OUT FOR MULTIPLE CLUBS CHOOSE THE MULTIPLE CLUBS TAB.

6.  You will then need to choose “Junior Membership” as your membership type.

7.  Next fill out the “Parent/Guardian Information” with information of who will sign your form. Then hit “Submit” at the bottom of the page.

8.  The “Confirm” page will then ask if the information is correct. If something needs to be corrected hit the “Edit” button next to the section. Once all the information is correct, go to the bottom of the page and hit the “I Agree” check box.  Then hit the “Confirm” prompt. You will be asked, “Are you sure you want to submit your application”. If you want to submit your information respond by hitting “OK”.

9.  The next page will state “Your Membership APPLICATION has been successfully submitted”. Print this screen for your records. Press “OK” to continue.

10. At the bottom of the page there will be a “Waiver & Code of Conduct” and “Player Medical Release” Form. Click on these forms and at the top of the page click on “Click here to Print”.

11. Once you have a print out of the “Waiver & Code of Conduct” and “Player Medical Release” Forms you and you parent/guardian will need to sign and date the forms.

12. All forms (Waiver & Code of Conduct, Player Medical History Form) must be completed, signed and submitted to the Junior Club Director at tryouts. These forms can be copied if you are attending multiple tryouts. After being accepted to a club, the Club Director will submit your forms and fees to the Region for the year.

13. You will receive via email an “Application Confirmation” stating the following:

 

                                 Membership Type:    Junior Membership

                                          Status:                   Unpaid

                                          Membership Dates: 11/1/2008 – 10/31/2009

                                          Region:                   Palmetto Region


DO NOT SEND THESE FEES TO THE REGION OFFICE – THE CLUB WILL
TAKE CARE OF THIS FEE FOR YOU ONCE YOU HAVE BEEN SELECTED!


 

14.  Once the Region office receives your signed “Waiver & Code of Conduct” form and membership fee from the Club Director, and confirms the member ship, you will receive another email “Membership Confirmation” which will give you your membership number and states the following:

 

                                 Membership Type:    Junior Membership

                                          Level of Play:          J6

                                          Status:                   Paid

                                          Membership Dates:  11/1/2008 – 10/31/2009

                                          Region:                  Palmetto Region

 

15.  If you need to update any of your personal information all you need to do is Log In at;

Then “My Information” and change your information.

 


The Palmetto Region website has been undergoing a change in it's site design since the beginning of October.  We wanted to bring you up-to-date information and in a format that allows you to find items on the website easier by using drop-down menus and a standard look-n-feel as you move through the pages.  Now - we want to hear from you!  Send us an email letting us know what you think, make suggestions and tell us what you like/or do not like on the website.  Site changes will continue throughout October as we prepare for the upcoming USAV season.
